For your convenience and to make the registration process as easy as possible, we have made the following forms and information available online. Your registration in the doctor's office will take less time if you complete each of these forms before arrival and bring them with you. Please also bring your current insurance card, official picture identification and be prepared to pay your insurance deductible and co-payment as specified by your insurance contract. - Fax both forms to 770-287-7022 at least 4 days BEFORE scheduled surgery date
- A nurse will call you before your surgery with pre-operative instructions
